If you have also requested a clinical transcript and would like all of your documents to be posted together, then in the address to which replacement certificate is to be sent section, please write “Forward to Uxbridge Registry FAO Gareth Stoneman

Please note there is charge of £25.00 for a replacement Transcripts.  Payment must be in UK pounds sterling. Transcripts will not be sent out until payment has cleared
